About Us

The London Family Health Team is one of 184 Family Health Teams in Ontario. The team comprises a group of interdisciplinary professionals working to improve the quality of life of over 20,000 patients served through prevention, identification and management of disease. In addition to 16 physicians, the interdisciplinary team includes a nurse practitioner, registered dietitian, respiratory therapist, pharmacist, psychologist, registered nurses and social workers with expertise in specific areas of diagnosis, treatment and care. These allied health professionals help support the efforts of the physicians in providing a holistic approach to the health and well being of patients and families.

The London FHT also has a strong commitment to teaching and research and seeks to develop partnerships with teaching and research organizations and professional bodies as well as participate in training programs for physicians and allied health professionals.

The Physicians and Staff at the London Family Health Team are bound bylaw and ethics to safeguard your privacy and the confidentiality of your personal information.

We collect, use and disclose your personal health information to:

  • treat and care for you
  • get payment for your treatment and care (from OHIP, WSIB, your private insurer or others)
  • plan, administer and manage our internal operations
  • conduct risk management and quality improvement activities
  • share with your other health service providers for the purpose of care
  • teach
  • conduct research
  • compile statistics
  • comply with legal and regulatory requirements
  •  fulfill other purposes permitted or required by law

Your request for care implies consent for our collection, use and disclosure of your personal information for purposes related to your care as noted above. All other purposes would require your express consent.   You have the right at any time to withhold or withdraw your consent todisclose personal health information. You will be required to sign the appropriate form which will be forwarded to the Privacy Officer.
