What is Primary Care?

Primary care focuses on health and includes health promotion, illness and injury prevention, as well as diagnosis and treatment of illness and injuries.

What is a Family Health Team?

A family health team is a group of health care providers working together to help keep you and your family healthy and to coordinate the best possible care for your health needs. Family health teams include family doctors and other independent health professionals such as dietitians, nurse practitioners, registered nurses, social workers, etc. This team based approach offers you better access to appropriate care in your community.

What health professionals are part of the London Family Health Team?

The London Family Health Team includes registered nurses, social workers, a nurse practitioner, a pharmacist, a dietitian, a child psychologist, and a respiratory therapist. These interdisciplinary health professionals have expertise in specific areas and help support the efforts of you and your doctor in caring for your health.

I’m looking for a doctor. How do I find one?

Physicians at the London Family Health Team is NOT accepting new patients at this time. The waiting list is closed due to a backlog of applications.

Health Connect Ontario is a service that helps Ontarians find a physician. You can either call them at 811 or visit them online to register for their services. You can read more about Health Connect Ontario on our website.

NOT ASSOCIATED with the London FHT, Nixon Medical Clinic (510 Southdale Road) is accepting new patients. Six new physicians are starting in Feb 2024. Applications are available at their Front Desk.

I am not a London Family Health Team patient. Can I access the After Hours clinic or the services of the allied health professionals?

The services of the London Family Health Team are only available to patients rostered to our doctors.

When should I use the after hours clinic?

If you have a cough or cold, fever, sinus infection, urinary tract infection or skin rash or irritation which you feel cannot wait until a routine appointment with your Family Physician, then the After Hours Clinic is able to assist you. The After Hours Clinic is not meant to replace the complex and continuing care that your Family Physician provides.

Do I need to bring my health care to every appointment?

It’s necessary that you present your health card to the front desk at every appointment in order to receive care. You must also ensure that your health card is not expired.

Is there a cost to using the services of the allied health professionals?

  • The services of the allied health professionals are covered under OHIP (Ontario Health Insurance Plan). Please remember to bring your health card with you for your appointment.
  • For some specific programs that may include additional materials for your own use, there may be a small fee for these additional materials. For example, if you participate in a nutrition program that offers cooking demonstrations, there may be a small fee to cover the cost of the food provided.

Is your office wheelchair accessible?

Our office is wheelchair accessible. We strive to provide access to programs and services for patients with disabilities in a way that respects their rights to dignity, independence, and integration.

Is your office on a London Transit Commission bus route?
