Nurse Practitioner

A Nurse Practitioner (NP) is a Registered Nurse (RN) who is licensed by the College of Nurses in the Extended Class.  An NP is an RN who has completed a master’s degree in nursing, and additional advanced university education. This education enables them to work both independently and collaboratively in an advanced practice role with other health care professionals to provide individuals and families with high quality care services.  NPs take care of the physical, emotional, mental, and social aspects of their patient’s health needs.

The Nurse Practitioner provides the following services to patients of physicians within the London Family Health Team:

  • Improves clinical care of patients with complex chronic disease through assessment and management
  • Provides access for patients to interdisciplinary health care professionals on the team
  • Performs full cognitive assessments
  • Orders and interpret diagnostic tests such as ultrasounds, x-rays, and blood work
  • Prescribes medication
  • Develops coordinated care plans with the patient and the care team to effectively meet the patient’s needs
  • Offers Advance Care Planning to complex chronic disease patients and their caregivers
  • Provides access to specialists such as a geriatrician, endocrinologist and psychiatrist as needed

Nurse practitioners work both independently and collaboratively with other health care professionals to provide individuals and families with high quality care services. The London FHT NP focuses on complex chronic disease management. The NP can prescribe medications, order and interpret diagnostic tests such as X-Rays and bloodwork, and develop care plans.
