Registered Nurses

Registered Nurses within the London Family Health Team work with our doctors, social workers, dietitians, and pharmacists to provide care that promotes health and prevents disease.

Nurses at the London Family Health Team work with patients to assist with the management and prevention of chronic disease through patient centred care. They assess patients’ needs and work with them to develop a plan of individual care emphasizing the patients’ central role in managing their illness. This work complements traditional patient education by promoting good lifestyle management to support healthy outcomes and helps patients achieve the best quality of life with their chronic condition.
